Anything you can roll, or carry onto the BTS you can take with you. 2 bags each is about the limit of what you can travel with easily.
The VIP bus to Koh Chang from Ekamai is bt280 each, and that includes the ferry RT ticket.
Coming back you can take the minivan at bt220. Everything is real easy to do to get there, and back.
You make one change to a songtaew going to the island, and no changes coming back to Victory Monument in Bangkok right by the BTS station, or you can take a taxi to your hotel after you get back.
Personally if I was bringing a GF with me, I would not stay in the lower Sukhumvit area to show her all the places you've mongered. I would stay closer to, or on the river to better show her the sights of Bangkok (using the river taxis), then maybe venture up to the lower Sukhumvit area one night so she can see it, but I wouldn't stay there every night. There are allot more romantic areas in Bangkok than lower Sukhumvit.
Use Pricewatch.com, and you can probably get rooms at the Shangra-La for under $80 a night, 5 star on the river under $80 are the requirements you want to use in your search. They also have one of the best river dinner cruises. Just something to seriously thing about after you get back from Koh Chang.
If you want to impress her with a romantic holiday lower Sukhumvit is not how to do it. You might want to take her to some of the top dance clubs on Sukhumvit soi 11 (Bed Supper Club, or Q-Bar, or Climax), then maybe a tame show at NANA, but that's all I'd take a GF to in the Sukhumvit area, and only for one night.
You might think about spending an evening at the Vertigo bar, or maybe have the Seafood Buffet at Baiyoke Sky Tower on the 78th floor, then get the views of the city from the 86th floor. The Saxiphone club by Victory Monument, and some of the live Thai folk music clubs in the area.
There are so many better things to do bringing a GF with you, unless she seriously wants to sleep with other girls, or troll for other men. Think about it.

Hi Koolbreez, You have hit the nail on its head. I have made inquiries in Riverside Ramada Menang hotel etc already. You are correctly mentioned, it may not be right choice to stay in Sukhumvit,though we may visit during evening time once or twice on a specific purpose.Thanks for all your comments and suggestions.

Then at NANA plaza I'd sit at Big Dogs on the rail just as you are about to enter the NANA area on the left, so you two can watch the goings, and comings in, and out of NANA, then after a few drinks, venture up to Spanky's for the show, then Casinovas, or is it Obsessions to show her the full NANA experience. A mild show at Spanky's starting at 10pm I believe, then Casinovas, or Obsessions so she can see the famed Ladyboys. That will give her the Bangkok sex trade experience without freaking her out. The Ladyboys might freak YOU out, but that's beside the point. She'd get a kick out of that, seeing you hit on....lolol.
