Last year, I bought 1 pair of progressive bifocals, to correct my astigmatism and nearsightedness, and 1 pair of folding reading glasses to wear with my contacts, anti-scratch, UV protective for 5800 THB =~ US$140, 1 week delivery. ÝI had my prescription, but she would have done the exam for like 500THB. ÝIf not for the latest progressive lenses, it would have been faster...

The shop was Nettaporn Optical, right next to the BTS stairs at Rachathewi (E. side of the street). ÝNice lady, well educated.

One of these days, it'll be THREE eyes: Lasik and a monocle! (The monocle for reading, and Colonel Klink impersonations...)

My missus gets her glasses from a shop on Silom right at the base or near enough to the SalaDaeng BTS skytrain station. They are very good, reasonably priced, make sure they are correct and fit and all. I presume you will need your prescription for the lenses strength etc, otherwise I know they can check you there, but not sure if they charge for it or include it in the cost. Their service is excellent and I even went in there the other day to buy a cleaning cloth for my sunnies and they do not sell them but they gave me one.
They are on the opposite side of Silom that patpong is on and if you walk down the bts or from central shopping centre and head towards convent rd or California gym, you will see them on the left.
