It's actually quite simple, the way I see it (when it comes to restaurants) - a waiter or waitress are being paid their base wage for coming to my table, bringing me the menu, taking my order, bringing me my food, and collecting the bill.
Anything beyond that, is what I base the tip on - at least insofar as I apply this system in California.
If they check with me frequently and keep my iced tea refilled, that's a bonus; if they do so without asking, that's better; if they do so without me noticing, that's awesome.
If they have a pleasant personality, are enjoyable to talk to, and contribute to my enjoyment of the locality, that's a bonus. If they are a girl, are cute, and have nice tits, that's an extra bonus.
If none of that applies, or if they are unpleasant, or ugly, the tip decreases, obviously.

The one and only time I was in Hooters, the waitress was standing, but the check that she wrote on was on the table, forcing her to bend over at the waist for an optimal view of her tits that were falling out of her top. Of course, once you have been to BKK, it's not really that impressive.
