I'll try:
Most women I've spoken to re this subject have told me that they don't like anal, because it hurts their assholes.
Some women have told me that they don't really like anal, but they do it because they need the money .
The (general) opinion of the women with whom I've discussed this subject leaves me in no doubt that they dislike anal because of pain, but do it only for getting extra money.
I've absolutely nothing against a woman giving a guy an HJ, providing that its done in private.
I do remember that a woman, in Phuket, actually burst into tears, because a guy forced her to do a HJ on him in full view of others - I found his action extremely distasteful.

The simple answer to your question is that no normal, somewhat decent, human being would wish to do any such thing. However, your argument falls down on two key points:
1) Anal sex need not involve degradation and the infliction of pain (of course, as with vaginal sex, it MAY involve both). Like it or not, anal sex is now a fairly common practice in Western heterosexual relationships, including married ones. I am not going to elaborate on this - there is more than enough material out there for you to educate yourself on the subject.
2) It is simply wrong to suggest that working girls in the Thai P4P scene (at least the mainstream farang-oriented sector) have 'little choice' but to agree to a punter's request for anal sex. Most BGs will NOT agree to such requests, and this is not expected of them as a condition of employment in a bar or MP. Given your close acquaintance with Thai P4P, how can this have escaped your notice?

I have to disagree with the posters who think that anal sex is frequently practised in everyday sexual relationships.
This has not been my experience and most women I have known have no interest in anal sex.
I have only ever had one women who wanted anal sex and specifically asked for it.
Myself I have no interest in anal sex.
Having said that i don't consider it to be a deviant practice just something a lot of guys seem to like but not too many women IMHO

Acceptable (or 'normal') sexual practices have been very much a moving target in the West in recent years (I can't speak for other cultures). As a result, I think there are actually quite large variations depending on age group and locality.
Go back 25 years, and even oral sex (of both kinds) was considered marginal and somewhat kinky. Absolutely no-one would admit to anal sex in public.
There has been a huge shift since then. Oral has been just about completely 'normalised', while in many quarters anal sex is talked (and written) about freely and without embarrassment.
I think you'll find that heterosexual anal sex is now quite widely practiced among 20-somethings in places like New York and London. I imagine it is far less prevalent among middle-aged couples in the Midwest or the Scottish lowlands. It still belongs towards the 'progressive' end of sexual practices, but I would guess than in a decade or two this will no longer be the case.
Oddly enough, sexual practices in LOS are relatively conservative, I think. But the farang-oriented P4P scene (inevitably) tends to play catch-up with Western practices. Just about all the BGs now offer oral, which was certainly not the case 10 years ago. I'd be prepared to bet quite a lot than in another 10 years, anal will be far more readily available in LOS than now.
Part of the 'problem' with anal is that many people are still experimenting. Hastily or incompetently done, it can certainly inflict pain, and I suspect many women have tried it once with an incompetent or over well-endowed partner and decided it's not for them. But it is certainly the case that many women have learned to at least tolerate something they know their lovers enjoy, and in many cases have acquired a taste for it themselves.
