i think in Major tourist areas like Patong and Pattya there is no problem. In Bangkok you have to understand that a bargirl is a hooker and right now you are eating with a hooker. Sure they should shut up and take it beucase your are their guest but you know thats not going to happen in light years. Let her pick the resturant and she be happy and not going to get the looks.

try places where it is common to have guys taking hookers (and believe it or not, western guys stand out when they are with women who are obviously hookers. Women from beer bars, gogo bars and so on don't really fit in at places full of business people in the same way I wouldn't fit in well at a gay bar. Well, probably not.)
I am sure you would not want to sit with your mom while at the next table there are a bunch of crack whores at the next table canoodling with some old fart. So, it can be a bit awkward as a restaurant owner trying to get the "right customer" there. And most restuarants, from McDonalds to Le Normandie have specific ideas of which groups of customers are compatible and will make them money. Staff may or may not think their shit is good or not, but as a former restaurant owner, if you aren't going for the western sex tourist market, then it can be the friggin kiss of death on a place in volume. Same probably would apply for biker gangs or skinheads; some other people feel uncomfortable around that sort of thing. Like my mom for instance.
So... some suggestions for nice places:
- Por Goong Pao Ratchada - great seafood, and lots of hos around - Thai,Chinese,farang customers
- pretty much anywhere in Silom/Sukhimvit/areas outside of the hotels
- if an Isaan woman (and this is a guess, only some women from the sex industry are from Isaan) then some Isaan restaurant
- beer halls: Dawun Daeng, Yes Indeed, etc - not exactly fine dining but good food and lots of booze
- the most upmarket hotels like the Oriental; just go there to wind them up if that is important to you
Besides which, someone with little education and lack of knowledge about how to eat western food with multiple courses might prefer to eat somewhere a little less intimidating. So... ask them where they'd like to eat; the off the way places are far nicer anyway.

If she is your average thai hooker my guess is (1) she prefers bugs to lobster and (2) she feels as uncomfortable as you in a smart restaurant.
Incidentally it reminds me of an ex - it was her birthday - I asked her where she wanted to go for dinner. I wasnt met with the standard response, namely, 'up to you'. She said she wanted to go to KFC.
She did order the lobster when I took her to Sirocco though. Not that she ate it of course.

I've not had any hospitality problems with restaurants when dining with my TGF who is from Isaan (and dances around a pole occasionally). I think this is because she is quiet in public, dresses conservatively when we are out, and carries herself well. It seems the Issan accent / manner of speech is often the giveaway to other Thais - but I'm only kind of guessing here.
These girls cop some ridicule, snide comments etc that often we do not notice when where with them.
An example is the taxi driver who was talking mainly Thai to my TGF, when I noticed she was visibly upset. Then some English I could work out like "...where you from ha ha ha""....You Isaan ladeeee..." "Yes, you Isaan ladeeee..I know.."
When I noticed, I asked her was he a DICK HEAD, ensuring he knew exactly what I was talking about.
I then made sure I paid him in the smallest change I could find - counting out slowly, and then made a point of a 1 baht tip - I think it was 1 baht (which I am told is an insult), followed by slamming of doors.
This was the first time in my six tours I have been really 'steamed' in BKK, but my TGF took it in her stride, just saying "thank you my darling", and following up with some terrific 'my hero! sex'.
It was a small thing, but an education about tolerance in Thailand

I took a girl from an upstairs bar in patpong to a really flash restaurant once. She was one of the showgirls in the ping pong show.
Anyway, she wanted sticky rice and fried chicken in this nice restaurant. They did not have it, so she went out to the cart nearby and ordered it. She brought it back inside and I asked her is she wanted to put it on a plate. I did not mind paying for the sevice, but she said no and ate it from the bag.
I was extremely embarrassed and the looks we got from the other diners were straight at me and full of disgust.
But that was nothing compared to how she opened the bottle of wine.
