I use my Thai nickname when dealing with people other than my wife or her family and friends.
When in Pattaya, ask any girl in town if she knows "Somchai" the farang. You might be surprised at the number that do know me?  
That being said, when meeting other BTF'rs, they usually use my BTF handle when addressing me.

I use a version of my BTF nic. It stands out to the girls as most have never heard it.
Caught once as the girl looked at my luggage tag on my bag and knew I was bullshitting her.
Hell they use nick names why not use them.

A related question:
When I introduce myself as heiko (pronounced hai kho) some people would just call me "bicycle". I have asked about this some different Thai people, but none of them would figure out why heiko translates to bicycle. Right now I consider it to be an insider joke. So why bother ? As it it, I really AM a bicycle enthusiast (spelling ?). It would come to great honour to me if in any asian language heiko actually means bicycle.

I didn't realise that "real life" included time with BGs I thought that was either Fantasy or wishful thinkin'!
