Get on a flight with too many cabbage class passengers, some empty seats in business class, wear a suit (or at least look smart), shave and smile a lot. Arriving early sometimes helps. Being sober helps. Flashing an airline ID helps. A PPL or CPL may help. Having millions of airmiles helps. Picking up a cabin attendant working on the flight the night before helps.
Best thing is probably a fairy godmother.

Having the title 'Dr' once upon a time in front of my name used to help me get upgrades. But alas this is no more on the majority of airline I fly with. (Knew that PhD was a waste of time. )
You could make up a title such as reverend or Prince I suppose?

I've been upgraded a few times simply by asking are you offering a free upgrade to business today?
Another way that works wonders in Asia offer a bribe say $20USD to the gate agent.

MarkC post has it right on the ball in all aspects, I have been upgraded on about 40% of my flights.
Another trick I've used was a company I worked for had an in-house travel agent and as I often had to fly at short notice would pick up my ticket at the airport. The travel companys rep would meet me at the checkin desk and I would take her for a coffee before checking in. A quick slip of 50 pounds got me upgraded both ways.
I now travel Eva and have a silver FF card if a free upgrade is not likely I ask to be debited for an upgrade, the counter agent then starts to loose some face and I get the free upgrade.
In London just place a nice box of chocolates on the counter while you fumble for you passport and ticket, the girl will ask 'are these for me' well you know what to say next don't you!!
