Dude, what you're talking about are very personal issues about risk management that everyone needs to decide for themselves. So get as much info on the risks for each tempting activity and decide for yourself if it's worth it. It also depends a lot on your own self-control, in the heat of the moment, especially after some liquid refreshment your risk assessment skills can go out the window.
These are my opinions on certain risks, modify them for your own tolerances.
Kissing - almost all BGs will allow or initiate kissing, some light, some deep and sloppy. You can pick up a lot of germs and STDs this way. IMO, if you are selective, you can minimize the major risks.
BBBJs - to me this has about the same amount of risk as deep kissing. If the girls is worth kissing, then probably a BBBJ isn't going to give you anything new, so it's a good risk. If you wouldn't kiss the girl, then you might not want her lips on your dick either.
Muff Diving - this is a little more risky than the two above, IMO. Again, if you're selective you can cut the risks down a lot. If you really enjoy this and want the full GFE then it would be a shame to pass it up on a nice clean girl. Of course, you are the customer, so don't feel you need to do this to get the girl off (one reason why paying for it is better than getting it for free).
Hard Drugs - avoid at all costs, the risks are not worth it for any but the lowest rung on society.
Weed or X - up to you. If you are really into these and are very careful it may be worth the risk. For your first trip to Thailand, the girls alone will be so mind-blowing that you shouldn't really need these, though. If you choose to indulge, be very, very careful about where and with who you do it with. It's probably not too hard to find a few girls who will give you a few tokes and thus you can avoid ever carrying.
BBFS rarely worth it. Yes the extra sensation (and convenience for you) is great, but the risks are too high for most of us. But then you knew that already.
It's not a bad idea to get your Hep A and B vaccines before you go.

I think the differences come into play when we ,as individuals, assess the significance of those risks to private lives and families.

You're right, we don't live in a vacuum, our choices do affect others. But I disagree with your level of confidence on people knowing the risks and evaluating them properly. Most people don't make intelligent choices, even for themselves. They take too much risk, or not enough (e.g. many guys will think nothing of deep kissing a girl in a gogo, but get a BBBJ? that's way too risky).
The effects on others are, in most cases, much less significant, but still worth considering. Other than as a purely philosophical statement, I don't see how it applies here.

OK. I hope I am right on the risk assesssment issue, but my experience is limited to peer group debate rather then analysis.
Perhaps I should expand on the other point I touched on. To a single guy an STD may be little more than an inconvenience in having to get the relevent treatment. But to a married guy who goes home with a runny nose and infects the partner may well see the situation differently.

That's the kind of situation I figured you were mostly referring to. That is definitely a concern for the married guy, but it is really hard to judge from outside the situation. But while we can't judge, we can certainly weigh in with our opinions 
I've seen all kinds of guys taking obscene risks with the health of their families and others who make more reasonable and rational choices. And even some guys who refuse to even get a lap dance because they don't want to offend their completely asexual wives. Some guys have told tales of woe about their wives being completely uninterested in sex and using it only as a bargaining tool. I have no problem with those guys getting BBBJs or 69 action while in Bkk. I would draw the line at subjecting the wife to any reasonable chance of HIV. İHell, even kissing a bargirl subjects the wife/gf to some risk (most likely just a cold or flu, but potentially something nastier).

I have noticed my attitude change as my circumstances have changed.
I was married but now divorced. I am still careful and don't ride bb, but I am now happy to enjoy bbbj. OK, some risk and I do have regular check ups. Never had a problem but it sure would have concerned me when I was married.
On previous threads when we got into this subject, it was very clear that some guys were looking for a level of comfort which tried to somehow justify that sexual activity outside thier marriage was a "minimal" risk.
It really is something for the individual to determine but I agree with a comment you made earlier that not everyone ALWAYS thinks with their brain rather than their balls.

Some guys do go out of their way to justify, what to me, is egregious behavior. But in some cases, it really is justified. It's hard for anyone to tell the difference.
It really is something for the individual to determine but I agree with a comment you made earlier that not everyone ALWAYS thinks with their brain rather than their balls.
But it's just as wrong to neglect those precious balls, too. Our entertainment, whether it is sex, sports, gambling, whatever has some non-zero value. If we all take no risk when there is the slightest chance of harming others, then we all lose.
