Any real doctors on this forum?

What is the best treatment when you suspect a Gonorrhoea that you catched-up in bkk?

It seems that some anti biotics that are listed to treat Gonorrhea do not work.

Back in the day, treatment of gonorrhea was easy. Fluoroquinolones worked fine. (Ciprofloxacin, Levofloxacin, Ofloxacin). Then, the bug got smarter and now there is a lot of resistance to these antibiotics. Current CDC recommendations are that, at least in some parts of the U.S., like California and Hawaii, ceftriaxone (Rocephin) and cefixime are first line therapy. Thing is, there is likely even greater drug resistance in Asia. So, the short answer is that if you come down with gonorrhea, and if you got it in Asia, then there is at least a 50-50 chance that the flouroquinolones won't work. Cefixime and Rocephin are what is currently recommended.

Been lucky, never had the big G since I've been coming to Thailand but got it in Vietnam during the war, twice. Quite an experience. The big G was rampant in Vienam back then and they said the same thing then that they do today in thailand, that their are strains resistant to cure and even "incurable" at times. I think "incurable" part was rumours started in an attempt at the impossible, to keep 19 year old soldiers away from the local pussy. There were 9 soldiers in for sick call that first time, all had the big G. The medic told us all to bend over, hands on the wall, and he went down the assembly line stabbing each of us in the butt with a needle with penicillin. Hurt like hell, but don't know why, probably not the needle as much as the penicllin reaction. Worked good though, one shot knocked it out, but christ did it hurt. The medic was laughing all the way down the line.

I'm curious, someone mentioned that they were given pills by their doctor. I've always heard that treating the big G with pills was a no no because due to the time and amount of medicine that actually attacks it is much less and slower than taken by injection. It takes so long to actually cure the disease and that it is just "suppessed" for a time, making the patient symptom free, and they are actually still carrying it. Not sure if any of this is valid but thought I would share it with the group for feedback. Always wear a raincoat!

Little Big man, first of all, I want to convey my deepest respect for all Vietnam vets. Thanks for what you did. I too am a vet, but a much more recent one. As for the shot. Yeah there was a time when the big G was sensitive to Penicillin. Ah, the good ole days. It's not so much that getting a shot versus taking a pill 'attacks' the bug faster. It's all about plasma levels. And depending on the drug and how it's designed, good plasma levels can be achieved with oral or injected drugs. The fastest and most direct is with IV drugs. The penicillin shot was given intramuscularly, and is designed to form sort of a reservoir in the muscle that distributes into the bloodstream gradually but consistently. You could take pills to achieve the same thing, but then you are relying on a 19 year old to take medicine on a schedule for two weeks. Unlikely. After the G became resistant to Penicillin, the Fluoroquinolones became the latest thing. For sensitive strains, a one time dose will do ya. But not in that part of the world anymore. Not even in California or Hawaii. Nowadays, the only thing that works consistently for resistant strains is Rocephin (another shot) or Cefixime, which was just discontinued by Wyeth labs because its patent expired. Oh, and the other good news about the G is that about 50% of the time, whoever is carrying it is co-infected with Chlamydia. So if you get one, you might have gotten the other. What a deal.
