A late update: I finally got my extension until March (unfortunately I leave in 10 days), and an extra re-entry permit which I already used - just got back from KL. This visa issue has been time consuming and a bit stressful, but with a happy end.
Thank you all for your guidelines, they did help!

I used this service yesterday. Great service - I'l use it again. However, a few clarifications are in order:
Service is called FineDay Tours & Trade. Their plastic passport sheath they left me with says "Visa Tour, Daily Cambodia"
The "bus station" was stated by the woman on the phone as being on Asoke, across from Soi Cowboy. That's roughly true. It was actually at a big iron gate to some compound, just up the way from directly across from Soi Cowboy. I didn't locate any actual bus station - I found the spot by noticing a lone farang standing on Asoke at 6:20 in the morning.
Given that I only had a phone number from this forum, I expected to find a minibus full of sweaty farang. Instead, I was greeted by a tour bus half-full of Japanese and Koreans. I thought I had the wrong bus; especially since they didn't have my name on the list, despite my having called.
Turns out there are Japanese and Korean expats that need to do visa runs too. Go figure.  There were 3 other farang on the bus too.
The bus was very nice, had the 2 TV screens (they played movies) and even had a working toilet. The cost is indeed 2000 baht, the lunch at the Casino was good, and they gave us one soft drink on each trip. No hassles at the border, the guides herded us around, gathering up passports and then lining us up for stamps.
They apparently have more than one phone number to call; the following is the listing I have:

I found it amusing that the guides seemed to speak more Japanese than English.

Edit: Let it be understood that their English was more than adequate, and I had no problems communicating.
