Never actually been in one of these strange establishments in Thai. But, in the good old days (late 1960's) I recall a rather respectable place just outside Vientiene in which the custom was for the lassies to go under the table cloth, open one's fly, and give good service. While this was happening the form was just to continue chatting, eat ones meal, and finish the wine. The lassies were extremely well trainéd in this regard.
Do any oldster brothers still recall the name of this rather unique cafe? Was it Madame Lilly's by any chance?

Slightly OT to your post, but the esteemed Darryl F. Zanuck head of 20th Century Fox in Hollywood in the 30s and 40s used to chair meetings in similar fashion as you mention with the starlet du jour under the table.
Was in Vientiane last month but, sadly, did not encounter this practise. Standards are deteriorating all over it seems.
