1. SOL (the Star of Light) on PP2
Far above all others. You can say what you want about its seediness, but all things considered, the girls are just the BEST in the business, they know it and their customers know it.

I can't really comment on Pattaya, not having had an extensive range of experience there, but 'My Friend You' bar comes to mind.
My first three choices of Blow-job bars in Bangkok is not so difficult to decide and is the fruit of many years of extensive personal research.
My First choice would be The Star of Light on Patpong 2.
