1. How important is décor and what aspects of décor impress you?
a. Seating
Seating would be good, yes. Nothing fancy required. The toilet-seat style barstools at Mandarin, NEP are a perfect example of what not to do here.
b. Lighting
Girls look better under UV lights as it hides blemishes etc. But girls without blemishes would be nicer, eh?
c. Dance Floor
No thanks.
d. Toilet Facilities
Yes please.
2. How do you like your music
a. What type of music do you like?
Anything except the usual techno dirge and cheesy rock would be a nice change.
b. What is your ideal music volume?
Not so loud that I can't have a conversation (unfortunately the case in 99% of bars).
c. Do you like to request your favorite tunes?
Are you going to pay a farang to DJ? Thai DJs usually won't speak enough English for this to be workable.
3. Drinks
a. Do you drink imported beers (if yes please state brand) ?
Yes, Erotica at NEP do a terrific Belgian wheat beer called Hoegaarden. Get some. Beer Laos is also excellent, if this counts.
b. Do you drink from bottle or a glass?
Bottle, I'm still under 40.
c. Do you drink spirits (if yes please state brands) ?
No.
d. Do you drink wine in bars (red / white) ?
Not in Thailand, haven't found anywhere selling decent wine at a sensible price.
4. Facilities
a. Would you use a free internet facility in the bar?
Only in an emergency (danger from keyloggers, etc)
b. Would you use a cheap fee based internet phone in the bar?
No, I have one at home without the background noise.
c. What type (s) of TV channel would you like to watch?
Soccer - sorry guys.
d. Is an in-house ST room important?
Only if it's nicer/cheaper than the nearest alternative (none of the ST rooms on Cowboy are nicer or cheaper than Asoke Place for example).
e. Is an in-house photo studio of interest?
Possibly - more details required!
f. Do you like to play pool?
Yes, but having to barfine the girl in order to play with her is insane (see Shark bar on Cowboy)
5. Services
a. What in your opinion constitutes impressive service from behind the bar staff?
Prompt service, correct bill, not begging for tips or drinks.
b. Would you like an option to pre-buy drinks at a discount?
Can't hurt.
c. What bar snacks or food would you like to see available?
Can't go wrong with burgers and fries, Cathouse at NEP will order in pizza too which is a nice touch (but not cheap).
d. What other services are important to you?
Nothing springs to mind. Keep it simple, but do it well. Provide cute girls and reasonable prices, and you will do well.
6. Hostesses & Dancers
a. What age range is acceptable for your companion at a bar?
30 and below please, I'm still in my prime!
b. What weight range is acceptable for your companion at a bar?
I like 'em skinny, up to 45kg at a push I guess.
c. Do you prefer to be approached or approach a hostess in a bar?
I don't mind being approached, whether I'm interested or not. What I mind is when they immediately beg for drinks or refuse to take a polite 'no' for an answer.
d. With what level of English are you able to enjoy the company of a hostess, none, basic or good?
Any - my Thai is just about good enough.
e. Would a discounted weekly BF rate be of interest?
Potentially, but you'd have to have a seriously breathtaking girl. But if I wanted to spend several nights with her I'd probably just exchange phone numbers and have her come to my place after work - and dodge the barfine. Sorry! 
7. Concepts & Themes
a. Do you enjoy Uniforms, Schoolgirl, Nurse, Student, Latex, etc. or prefer Bikini / Nude?
Yes to all of the above.
b. Do you like shows? (if yes please state preference, lesbian, ping-pong, etc.)
Prefer the erotic to the grotesque. Sheba's on Cowboy have it about right, when the boys in brown aren't looking.
c. Do you like table dancing?
Nothing against it. Quality of girls is more important than format though.
d. Do you or would you enjoy a lap dance?
Gogo girls will generally sit on my lap anyway. Paying for an organised version doesn't particularly appeal.

a. How often do you come to Thailand?
I live here.
b. Would you travel to Soi 6 for a good bar?
Sukhumvit soi 6? Rachada soi 6? Silom soi 6? Pattaya?
9. Comments – Please give additional input, insights etc. here.
Put simply, get a bunch of attractive girls, price your drinks (and barfines) reasonably, don't let the girls hassle the punters for lady-drinks, and you will do well enough (assuming you're in a vaguely decent location). Gimmicks can help if they're done well (the photo studio and outfit/shows are good), but only in addition to the above.

Perhaps so. Though most people in sales and marketing would disagree with you. Most companies spend a lot of time on surveys, needs analyses, and customer feedback. Businessmen need to make a profit, and look for ways to make their own "vision" adapt to what is profitable. They are not omniscient or existing on unlimited funds, so this kind of thing is regarded as an essential part of trying to get a "good idea of what is needed by the customers".

Sure, standard stuff, but I agree with Oaktoad.
You can ask questions all you like, but here there is no profile of who is responding - not age, education, nationallity, local, visitor, whatever......
The questions are interesting but far too broad - the answers are useless without a respondant profile to categorize and define the responses to possible customers.

exactly, trying to please anyone is gonna please no one, those few succesful bars (and sucess isnt alot of guys hanging around drinking one beer @ 90 baht every 240 minutes) I can recall are firm in their concept, and deaf ear to all those guys who always find a hair in the soup. good luck anyway

He would be better off going around and visiting some bars.
He could see what they do and take the best aspects to incorperate into his bar.
He must make sure that he only visits bars that service his target market and are making some money. Too often someone creates 'the perfect bar' and either prices out the market or goes under from the lack of cash flow...
