Sars confirmed, tally of cases grows
Anhui victim travelled by train while ill, and her mother has died - prompting fears of human-to-human transmission

The mainland confirmed two cases of Sars yesterday and reported another two suspected cases - including a woman who died this week.

One of the confirmed cases is a medical student surnamed Song, from Anhui province, and the woman who died is her mother, surnamed Wei.

Wei's body was cremated yesterday afternoon. The second confirmed case was identified as Li Na, a 20-year-old nurse at Beijing's Jiangong Hospital.

The Health Ministry said last night the Sars cases may have resulted from a laboratory infection.

Investigations also showed the cases of Ms Li and Ms Song were linked, a spokesman said. Ms Song had been treated for viral pneumonia at Jiangong Hospital and Ms Li cared for her there from March 29 to April 2.

The other suspected case is a 31-year-old man from Beijing, surnamed Yang. Ms Song and Mr Yang had both worked at a Beijing laboratory for the Centres for Disease Control (CDC), Xinhua said.

Ms Song studied at the Beijing laboratory from March 7 to 22. She is believed to have infected her mother. Ms Song's mother was admitted to hospital on April 8 with a fever and an unidentified pneumonia-like virus. She died on Monday.

The women took several train journeys together between Beijing and Anhui and may have exposed many other people to the virus. Hospitals along the rail line have been put on alert to report any cases of pneumonia.

"When the daughter was ill, the mother accompanied her all the time," the Health Ministry said on its website.

A doctor who treated Ms Song said the woman was allowed to travel back and forth despite her symptoms because people thought Sars was no longer a threat.

"At that time the alarm system about Sars had stopped and we thought Sars was gone," he said.

The World Health Organisation is considering sending a team of experts to the mainland to help trace the women's movements over the past few weeks.

"It looks like we had human-to-human transmission and there's clearly a travel history where they might have exposed other people," said Maria Cheng, a spokeswoman based in Geneva.

Mr Yang, a post-doctoral researcher, has had fever since April 17 and was admitted to Ditan Hospital on Thursday.

The Health Ministry has sealed off the lab where Mr Yang and Ms Song were working and other staff members have been put under medical observation.

Temperature checks have been introduced at all mainland ports of entry and a mass disinfection of public areas has been launched.

"Anyone who has a temperature over 38 degrees Celsius will be taken to hospital," said a Health Ministry statement published in newspapers. "No one will be exempt."

A total of 117 people who came into close contact with Ms Song and her mother have been isolated. Another 188 people who had close contact with Ms Li were previously isolated. Five have fever symptoms.

The WHO welcomed the identification of the source of infection.

"The fact that these infections can be traced to a laboratory, while disturbing, is also to some degree reassuring - the initial source of the Sars virus does not appear to be animal or community-related," it said.

"Continued close contact tracing and heightened disease surveillance and reporting are vital now to break any potential chain of transmission of the virus."

A researcher with the Beijing city CDC said safety measures at the laboratory should be stepped up, and was concerned about irregularities during the conducting of experiments.
