You may not want to visualize a sandwich literally. If you get two girls to service you at the same time, I think you have achieved a "sandwich". How and what will always be up to you and them. But be assertaive and expalin what you want beforehand.
I also would suggest to always ask the mamasan about a recommendation as to who will give you proper service and if they steer you right, give a tip to the mamasan after as well.

Had another very good session yesterday at Orchid. One of the things I appreciate is they have all the girls come out of the "holding pen" when you enter. At first this may seem little intimidating but I find it better than having to look at a book.
