She didn't kick up a fuss because the police in Phuket are pretty tough on massage shops offering extras. She was probably not from Phuket and thought you understood the BKK system and would automatically pay the standard surcharge discretely, so as to remain within the law, she didn't offer the extras explicitly. I think the reason why police are tough is because the tourist industry puts pressure on the local administration to keep the image of Phuket as clean as possible so it doesn't fall into the image of Pattaya, which the tourism industry has been decimated by the out of control sex trade.
