I've never seen a Lolita's girl 'expect' a tip, scowl or hold her hands out for one, like gogo girls often do so I wouldn't worry about creating a new defacto standard.
Tips are all over the place but, cheapcharlie's is in the lower range and there are very few guys who don't tip at all. I know one girl who keeps a book. Her average tip works out to around 300B - and more guys than you'd think tip a lot more.
Everyone's heard the usual arguments for and against tipping but I'd throw in another one here. The cuties get bigger tips than the rest. If they got nothing but guys taking ages to come but tipping peanuts they'd soon leave and go somewhere else. A blowjob is still a blowjob without a CIM so she may be within her rights. But it's implicit... as is the tip for doing it.
There are other things that, strictly speaking, a bj bar girl doesn't HAVE to do. For example: going topless; letting you fondle her breasts, etc. Just as you say "700B is for a blowjob", she could say "700B is for a blowjob, no more, no less" and be non-compliant in all those kind of things. She could also choose not to listen to you when you say you like it done such-and-such a way, etc. So there's plenty of leeway for tips.

Can you 'book' talent? I don't think you can make bookings unless you're very friendly with the owner.
One girl told me a guy who was very friendly with the owner and took a very long time to come used to call ahead to check if she was there. Another guy who was very good mates with the owner could supposedly order take-outs to his room but that's about it.

Unless it's changed in the last few months, the pricing's like this:
1. BJ - 700B
2. FS behind curtain - 1100B
3. FS in private room - 1600B
For those interested in the girls' cut,
1. BJ - the girl gets 300, the house gets 400
2. FS behind curtain - the girl gets 400, house 700
3. FS in private room - the girl gets 500, house 1100
