It might be useful to distinguish between a 'tip' and a 'payment for extra services'.
What I think the OP was asking about was: how much should you pay for the extra service. His take on it was, I think, that there is probably a standard rate for a handjob or whatever, and he wants to know what that is.
Then there is the question of how much should you tip for good service. Some cultures tip a lot (the US) others hardly at all (thailand), and others seemingly randomly (UK tips waiters and taxi drivers, but not bar staff).
I suspect that most people have their own idea about how much is the right amount to tip: what newbies (or not so newbies) are uncertain about is: how much should you pay for the very fact that the massage has suddenly gone all sexual.
Any views, other than those already expressed?

Great reading! A tip from me has to come straight from the heart. It tells my provider that I have been made happy by her or him for whatever reason. It could be a small act of smart thinking, or honesty, or a patient response to my queries, or readjustment to accomodate me.... So I might tip anyone. And, as some posters here pointed out, a bonus, a commission, and such like are analogous to the tip -- just like Webster's describes the clitoris as analogous to the penis in the male.
Once I bargained with a cabbie who demanded 350 baht to the hotel "because traffic." I insisted I would pay 300 and he reluctantly agreed. As it turned out, the traffic was bad, the guy was good, didn't pull a face when he got out my luggage and even smiled warmly and -- guess what -- I paid him 300 plus a 50 tip which I thought made him doubly happy.
Another time, at a MP near Central World the girl gave me one of the lousiest oilies I have ever wasted 1500 baht on and then coolly demanded a tip of 500! I smiled and expressed regrets. She pulled a face and I was far from caring because I was never ever going to step in there again.
I do tip the maids who maintain my room at the hotel because somehow they all seem so sincere and hardworking and I can see they readjust their rounds to let me laze longer (once a lovely maid even made my dream come true by entering without knocking when I was in the shower with the door of opportunity wide open --- sigh!) I tipped her 100 baht when I checked out. To me these are little fond events that make my trip pleasant and I express my appreciation to the unknown by these small acts which I hope would bring a smile in their hearts. A tip should be enjoyable as much to the giver as to the recipient. It is a perfect event if and when giver and recipient find themselves sharing the same vibe at the time the tip changes hands. In that respect a tip is a sacred way of being in communion with your inner self.
A critical part of the transaction is the receiver's reaction to the tip. Does she / he seem thankful and satisfied? To me that determines whether I have tipped effectively.
I guess, part of the reason short time visitors to LOS are in a bind over P4P and the associated costs is that we try and pack in too much of experiences within a short period. I figure I would enjoy it better if I were living here, sanooking at my own pace rather than winding myself up relentlessly every hour of one heavenly week as if it were my last. That mad rush syndrome inevitably leads me to a stage where I begin to feel, by the fourth or fifth day, that the cost-benefit ratio is getting skewed to my disadvantage.

There is one area where tips can go really crazy, and that is in the G-clubs. I for one, normally tip, 1,000 B to 1,500 B for the girls that spend their time with me. That's not including the lady drinks that they are already getting. Sometimes, if I feel good, the mamasan gets 1,000 B too, or else forget it. The toilet guy who gives me a good massage gets 100 B.  Oh yeah, and the waiters too, 100 B each. I have heard of girls getting 10,000 B tip for the night, and they don't even go home with the customer.
