Thailand uses 220V .. you can buy adaptors in the US. They are sold in many places like drugstores, travel stores (Rand McNally stores), etc .. You also need a different type of plug and they are also available.. You can buy the most of this is Bkk too.. check out some of the small shops on Sukhumvit .. It might be easier to buy the appliance in Bkk, depending on what how long you will be there or what appliance you are talking about .. also, battery operated is a viable option for short term stays.

I would do as Toad said. I have one and take it along to Europe and Asia. Works great. I only use it to charge my computer and camera. It also depends on what type of appliance you are going to use.

Those adapters/step-up/step-down transformers can be pretty heavy. I've found that a number of the higher-class hotels either a) have them available and will provide them to guests. b) have 110/120V sockets already in the room. I usually bring with me a small surge protector as a added precaution. Curtis or American Power Conversion makes these.
