Yes, the questions keep coming but what you will need to understand ( though I suspect you understand this already) is that nobody can tell you everything in a step by step guide format. Nothing pisses people off as much as someone coming back like a machine gun with constant micro detail questions that cannot be properly answered because every situation is different (and that should be a big part of the fun, finding out what works for you)
The obvious answer to your last questions are, in order,
Where do I flash my dollars? At reception, NO
to the mamasan? NO, (unless he/she was very helpful) Outside, to the girls? YES
During a first encounter, ST? Dont understand the question.
Find a girl you like and while relaxing with her after the deed, ask if she would like to spend the night with you, it's not hard to do, is it.

BTW, like most massage parlors, you can actually pay an establishment fee to take them out for holiday. Assuming that you already know each other, and have agrred to do so. The mamasan tells me that the take out fee is 1,500 B on week days, and 3,000 B on weekends (Sat & Sun). So, if you want to have a nice holiday in Koh Samui, for example, with your favorite girl, it is possible to do so.

How much would you pay a oil massage girl if you took her out for the whole day? If they have four customers, they would make 6k since they get to keep all of it unlike the soapy MP girls where they only get approximately 50%. I found out it costs 3k to take a girl out of Tulips and most MP places in Ratchada.

Good comments, but I don't agree about how do to a LT at Aviva. I would talk to Praew as she is the only mamasan at the mooment....i think. Mae is also there, but may not be the best to talk with about this.
I've done it a few times, but don't want to quote prices. The prices in the threads above are close. So, just ask.
If you're looking for "Annie" or similar quality, expect to pay. Or walk across to Nana Plaza if an LT is what you want and save the bucks.
