I've stayed at SGS a couple times but have never tried their spa. As such, can't give you 1st hand account, but I'll go out on limb and say I'm 99.2% sure its legit massage only. Being a 5-star hotel, I don't think the staff would want to chance losing their job (or future job opportunities) on providing extras to guests.

The spa is legit, but I wouldn't recommend it. The staff is poorly disciplined, being unable to stop chatting while the construction isn't well suited to a truly relaxing experience.
If you're just after a massage, go to a cheaper place. For a true 5-star spa experience, the Peninsula is the absolute class of BKK.
As for ID's, the higher end hotels cater to business customers, and they don't like security people interfering with their fun. All the global brands like Westin, Sheraton, Marriott, Conrad, etc... will look the other way, although I wouldn't recommend bringing a truly trashy BG into the Peninsula. There's some degree of standards you should try to adhere to, for your own sake if nothing else.

If you are staying at the Sheraton and looking for a massage with a happy ending, walk across the other side of Suk where the Westin is, and there is a massage place directly across from the Westin on Soi 19. Ask for the 1-2 hour Oil lube job. If they ask 1000 for the extra treatment, just say 500 and they will be happy.
I even banged one of the massage girls in there for free, but that is definately not the normal treatment.

Can see this thread has been inactive but title seems to be the most relevant.
Although the new overhead walkway that links Asoke BTS, Robinsons, Times Square and SGS is open, renovations continue in the SGS in the ground floor Rivas and convenience store areas. This has been the case for some time however last week I found the corridor to the ground floor lifts has been blocked. Not sure if it will reopen.
Outcome of the current situation is that the SGS walk of shame now involves the lobby. Seemed to be far less BGs about last week in the hotel although the couple I did see where not being hassled.
Still a great hotel.
