What exactly happens at a blow job bar, how does it work, and how much do you pay?
Having been asked these questions by a respected veteran of the forum, I thought I would attempt a little summary for the benefit of other "newbies". The answer of course differs somewhat depending on which particular BJ bar we are talking about, and the issue of tipping, in particular, has been hotly debated since ancient times in the BJ threads. I do not presume to offer a definitive statement on any of these hot topics (nor do I want to reignite old arguments). I view this more as an attempt to offer a general, "Lonely Planet" guide, if you will, to the basic anatomy of a blow job bar, for those without the time to read through the hundreds of pages on Lolitas or the Star of Light (SOL).
What happens at a blow job bar?
You get blown. Pure and simple. That's the beauty of it. That's what you go there for, and that's what you get. It is P4P fast food. 
That said, full service is often on the menu too, at least with some of the girls, but that has always seemed besides the point to me. The greatness of a BJ bar is that you don't have to wonder about whether the service provider is going to be reluctant to give you a nice long blow job, and keep on sucking until you are finished. All will suck you for at least 15-20 minutes without complaint, and some will go pretty much as long as you like (and are willing to pay extra for), but I generally try to keep it under a half hour from the time her lips touch my cock and balls to when I gently deposit my cum in her mouth. So, you might ask, is that all there is to it? Well, what more do you want? And two will generally allow you to extend the time, and even the number of pops, if that's your thing.
How does it work?
As you approach the bar the girls are typically sitting outside and you have a split second in which to make a choice, make eye contact, and with a little nod of the head, motion her to follow you inside. If you don't, the next one in line, so to speak, will follow you anyhow. But you can turn her down if her lips aren't to your liking and pop your head back outside for another look. At SOL or the Pump Station (in Pattaya) the girls may be inside or within view, allowing you to sit for a bit and survey the talent.
(One of the reasons I like the Pump Station and the BJ bars next to it, is that you can hang out and drink and eat as long as you like, before deciding to have a BJ too, or not. Most BJ bars are not really places where you want to hang out, although I do see guys playing pool at Lolitas in PTY).
Having made your selection, you can go straight upstairs without breaking stride, with or without a drink, or you can sit at the bar for a bit and seize up your service provider before you decide to close the deal. Generally, though, there isn't much to figure out. So, if the bar charges me 300 baht for the privilege of going upstairs, and my beer is 85 baht, I will give the girl a 500 baht "tip", figuring she doesn't get much of the house charge, if anything. But if the bar charges me 700 baht for the BJ, I will generally tip the girl 100, to bring her take closer to 500 baht, which is what I think an average BJ deserves.
As for tipping, as noted, there are a lot of differing views out there. So this is just my take on it. I don't consider paying the girl 500 to be tipping, but I also think the "average" BJ should include some breast play and cum in mouth (CIM). If the girl was exceptionally hard working (or I took longer than normal), then I might give her one or two hundred more (especially if I was planning a return visit -- I used to overpay a regular at the Pump Station, and it was well worth it).

As for your injury and question, as I am no doctor I can give you only the most inexpert opinion. farangman4, not sure what pricing at Arkane you are referring to. My usual selection, S3, 90 minutes of "fashion" massage, is 1580 baht.
