Not all - the watch I bought on my first trip 5 years ago still works, is accurate, and is waterproof.
The caveat is that it's a hit or miss, and unless you know what you are looking for, ItaliaThug's suggestion may be a good one.
Of course, if your friend is just looking for a fun fake, the $35-$60 a watch you pay in BKK are hard to beat. I bought 3 watches back then, as insurance, but never needed any of them.

Grade A fakes can be had for 4,000 baht in Bangkok. ok, they wont do some things like the originals (200 meters underwater for example)... but besides that, not even the pros would be able to tell.
