Probably states plus the dfference in price between what you paid and the fare for the day you want to fly. Sometimes each day has it's own airfare.

NW? Dude your screwed! You have a choice to either go standby or payup the differrence.If I was you I would simply buy another RT ticket on another carrier such as TG or SQ and rebook the NW ticket for a later date.
If geting out of BKK to transit thru NRT is the main problem, find out if you can go thru SIN. I have done that a couple of times when my fare basis was not available to NRT but had plenty of availability thru SIN.
Also look to change NW if u plan on going to Asia more than 2x a year. Go UA or CX or BR (EVA).
NW sucks big time I left them 4 years ago and never looked back. Their FF program really sucks too.

I think we might be hitting the limit of what this forum can suggest for your specific situation, but there are other resources. Seriously consider doing a few google searches and also look at these sites for more detailed suggestions.

You make the assumption that whoremongers that travel +15k miles per trip to Asia don't live and breathe airline rules? I think I have learned a thing or two flying +125k every year.
I agree with u that the other forums are an invaluable source of info. I happen to me a member to those myself.
Can always learn something new.
However to be quite frank the guy has very little wiggle room on a change for a return portion of a deep discount economy ticket.
NW is not going to cut him any slack to delay his trip by a day especially on an el cheapo fare. I doubt if even the medical excuse will work especially when you consider he has to buy up to another fare for at least $1000. That tells me economy is almost sold out for his return date.
I bet the problem is the BKK-NRT segment, that one is always a killer.

I called United and they told me if I had a medical emergency to call the reservation office in the US and talk to a supervisor and they would get me out at no additional charge as long as I had a note from the MD. They also said it might take a couple of days to get me out.
Different airlines, different rules. Check carefully. It's not unknown for airlines to interpret the rules differently from day to day.

oh well.. after all.. it's just one more day of my 3 weeks trip.
It seems it's little too risky to use other tricks mentioned earlier according to jjsushi.
I'll just try to change my flight one more time once I arrive in bkk and if the change fee is still too high, I'll just use my scheduled flight. I can't take the risk of standby because of work.
I personally don't like NW either but the price was unbeatable at that time. I paid less than $800 for this round trip ticket.

Yeah no charge to change the ticket makes sense for a medical emergency but they still have to find you a seat at your fare basis, which is why it would take a couple days to get you out, unless u were willing to go standby.

I'm fling Eva Air to Jakata with the ticket booked direct on their web service.
Change date of out or inbound. Free.
Cancell ticket. NT$300, US$10.
The business class ticket is NT$22,000. US$687.5. about half the price of the published fare. Booking on-line gets you 1000 extra air miles, with the 7 tickets I've bought this year on well on the way to a freebie.
I like to fly in comfort and have the security of being able to change flight times and dates to suite me.
If you can only afford the cheaper discount tickets I'm affraid you must take the deal.
Please also make sure you have good travel insurance that will cover repatriation flights.
I had a girl friend visit me in Canada who had to get home quick as her mother fell critically ill, she called the insurance company. They checked out her request but in the mean time they booked her on the next flight out, so that they only had to confirm the booking.
