Keep in mind that for a long time AIDS was not recognized because it was hidden by other diseases that were listed as the actual cause of death. It was not until technology had improved to the genetic level that 'T-Cells' were discovered.

So are all the folks in Africa dying of AIDS? Yeah because that is the level that our technology can deal with. Is there another transmission mechanism for AIDS? Possibly as most scientists are focused on the blood connection and are not looking for another.

The blood connection does not mean that sex is the only way to transmit AIDS either. Drug use, unsanitary needles in immunizations, transfusions with contaminated blood, and even tattooing with unsanitary needles could conceivably transmit AIDS.

The good news is that if there is another mechanism for transmission it has not yet left Africa ...

SW, the clinic is legit (I've been there before for minor ailments and been treated well), and very professional looking (separate rooms, nurses, scrubs, etc). The test procedure was also legit and yes, they showed me that they were opening up a brand new kit, so no worries there.

I am more concerned with the validity of the test due to the time of result and the price of the test. All this is based soley on comparison of tests have I heard of (so not a strong basis for my concern). Like I said, I have heard of the PCR tests but thought they were more expensive, however, this is Thailand where the cost of HIV medication is extremely cheap (thanks to a ground breaking case a Thai Dr/Scientist launched against the pharmaceutical corps which in essential allows for generic meds), so it would make sense that testing has gotten extremely cheap too. The other thing is that no decent/reputable clinic would actually administer fake tests and send people out there with fake results (especially with something as deadly as HIV), would they?

Either way I was tested at home before my trip, tested while I was there, and after sweating out the next 5 weeks, will get tested again just to close out the trifecta!

That is assuming that they are decent/reputable. The con man you least suspect is the one who looks like the guy next door.

As it is, many clinics are sending people out with fake meds as they do not always vet their source so if the clinic was depending on the results from a fake or ineffective test kit then you still lose.

Best bet is to go to a clinic that has known and trusted references...

The test you described is very much like the procedure at my local community health center in California. Five years ago, they'd send off a blood sample and you'd get results in a week. More recently they do the test while you wait. I believe the kit can also be used with saliva, but a drop of blood was used for me. The blood sample reacts with chemicals and the resulting colors indicate infection. The test I took measures anti-bodies to the infection which may not form until six months have passed. It's not accurate immediately following infection.

Like you, I've also heard of PCR tests, but have never had one. I've heard it's quicker, detects infection earlier in the cycle, and is more expensive. In my simple minded ignorance I'd assume the PCR test would involve a diagnostic machine, and a longer wait, instead of a chemical reaction.

Thanks for the reminder. It's almost been six months since vacation.

Sir Van Koff
I'm sure it's been said before, the PCR (DNA/RNA) test is not a diagnostic test. It is used only in confirmed cases for monitoring viral load and on infants because they might not yet have started producing antibodies. PCR has a high rate of false positives. I had one done and the wait almost drove me crazy. Also the possibility of being false positive. PCR is often prescribed just so the hospital/ lab can make a good profit. I think the testing / equipment is licensed from Ranbaxy and is very expensive. More tests justify the investment costs.

Remember, even the CDC in the USA does not accept PCR as a conclusive test. It has to be confirmed by an ELISA/EIA Antibody test followed by a more sensitive Western Blot (which is also an antibody test).

The regular antibody tests (3rd and 4th generation) are >95% accurate at 6 weeks and almost 100% at 3 months. The CDC in the USA has reduced the 6 months wait to 100% accuracy, now to 3 months.

Here in the Middle East most testing kits (Vidas Duo/Ultra) are 3rd/4rth gen ones manufactured by Biomeirux France. The whole thing is automated and results are available in approx 4 hours. The clinics / labs here do not recommend rapid testing due to false positives (defective kits). Costs about Baht 3000 here. As an expat, being tested positive means immediate deportation.

Hi Jingjo,
The risks are substantial and shouldn't be taken lightly but I think that being concerned about nail clippers, hairbrushes etc is a bit extreme. I'm also not clear that a connection has been established that kissing a girl that sucked a bunch of cocks makes a difference. She is either infected or she isn't and kissing hasn't been shown to be a significant risk. All that said, even oral sex man on woman, while a risk for many ugly things from gential warts to herpes is not a major risk for passing HIV unless all the conditions are right. My info may be out of date but I don't think so.

Hi Jingjo,
Yes, I am an American. I am not sure I know how that impacts the discussion.

I did not think you flamed anyone or were a prick. And by the way, we drive on the right, not the left.

I'm sorry to hear about the Hep C. That is not a good situation, but the topic was HIV and I was just suggesting that nail clippers, hairbrushes etc. are not a risk for HIV. As for girls giving even 6000 blow jobs, I think unless they have contracted HIV or even Hep C, that you cannot catch it from them. They have to have it first, one blow job or many. I'm not trying to argue, just to be clear.

As for the risks for Hep C, I have to admit I'm not sure of all the ways that it can pass. I was under the impression from things I have read that it is more likely passed by needle sharing and less from sex but maybe that has changed. I think Hep B is easily passed by sex. Anyway, I'm not an expert in this area, just wanted to be cautious about what we say about HIV transmission. We need to take care of the real risks and not the tales.

Wishing you well. Lets not let different use of language make us think we are flaming each other. That is not my intent either. It makes me think of the old saying "England and America (or in this case Australia and America), two countries separated by a common language."
